Lebanon Parents As Teachers Participation Agreement and Consent for Services
What is Parents As Teachers?
Parents As Teachers is an early childhood home visiting program designed to help you learn more about parenting, help support your child’s developmental needs, and help with the challenges of parenting. Our program services prenatal children through Kindergarten entry. It is a voluntary program, and has no cost associated with enrollment. Parents may choose to participate with monthly visits, visits every other month, attend monthly events, or simply receive informational emails.
Visit Model
Through our program you will have personal home visits with a certified parent educator to provide developmental activities and to help support your role in parenting. You will also have access to monthly group learning and group connection activities, as well as our resource network. Your child will receive a developmental and health screening within the first 90 days and again on a yearly basis throughout enrollment. Your family will also receive a family-centered assessment during your first 90 days.
Our educators are NOT certified medical professionals, diagnosticians, or mental health professionals. We will not diagnose your child. If you have a concern for a developmental, medical, or mental health delay our educators will do their best to provide you with resources to help connect you with qualified professionals.
Visit Expectations To help create a successful visit we ask that you do the following things Be present and
- Participate with your educator during the activities
- Turning off electronics (tv, phone, tablets, etc)
- Having pets contained as to not distract from the visit
- Engaging with your child and the parent educator during the visit.
- Share observations of your child with your educator, along with any important family information that may affect your child’s performance or stability.
Visit Cancellation
We understand that sometimes unexpected events come up and that you will have to cancel a visit from time-to-time. We ask that you do your best to contact your educator as early as possible to cancel and reschedule a visit. A family that cancels or misses 4 consecutive visits will be dismissed from the program. If at anytime your educator feels your cancellations affect your standing in the program, your case will be reviewed by the coordinator and director for possible dismissal.
Confidentiality
All Parents As Teachers records are stored digitally through our data collection system, Visit Tracker. Some educators also maintain paper records which are locked in a file cabinet in the Parent Educator Offices located at Esther Elementary. All biological parents and/or legal guardians have legal access to any and all files maintained through Parents As Teachers services. Files can be requested by contacting the Parents As Teachers Coordinator at 417-657-6002. Please allow 48 hours for all requests to be filled.
From time-to-time, Parents As Teachers records may be accessed by the Missouri Department of Education and/or Parents As Teachers National Center for reporting purposes.
Media Release
Participants in Parents As Teachers will be featured from time-to-time in media, including, but not limited to social media, newspaper, and television. Any parent not wishing to be featured, or not wishing to have their child featured in media may opt out by contacting pat@lebanon.k12.mo.us with a written request. Written requests must include the parent/guardian’s name(s) and child’s name(s), and must state that you do not want your child featured in the media.
Mandated Reporting and Educator Safety
As members of the Lebanon R-III School District, Parents As Teachers Educators are considered to be mandated reporters. If at any time your educator feels that your or your child’s health or safety are in danger they are legally obligated to make a report.
If at any time during a visit, an educator feels unsafe they reserve the right to end the visit without notice. Any further visits will be evaluated as to safety. If the educator feels unsafe in your home and you wish to continue visits, you will be required to meet the educator at the Lebanon Parents As Teachers facility located at Esther Elementary.
Program Review
All families participating in Parents As Teachers will be asked to complete a yearly survey to evaluate both the parent educator and the Parents As Teachers program. Results will be confidential.

By signing this form, I agree to allow the information described in the Confidentiality section to be collected and kept in the Parents As Teachers program. I understand that at any time, I can let my educator or coordinator know that I no longer wish to participate in services.
